In 2016, the Northwest Health Foundation will provide funds for implementation grants only. These projects should be ready to implement a plan of action, with goals and outcomes that address the social determinants of health in a community. The Foundation focuses its work in the areas of early life, educational attainment, and economic opportunity. The Foundation has specific funding regions in Washington and Oregon, see website for details. Implementation grants are available in amounts up to $200,000 for up to three years. Eligible organizations should submit letter of inquiry by May 6.
