GO Dough Fundraiser Request Form

Please fill out the form below to request your online fundraiser page and GO Dough card. For general information about GO Dough, click here.

  • Your GO Dough card will be shipped to this address in 2-3 weeks.
  • ex: Wellness curriculum ($100), New PE Equipment ($300), Fresh fruit for taste tests ($50), Wellness posters ($30), Professional Development ($300), etc.
  • GO Dough schools keep 75% of funds raised. To calculate your fundraising goal, multiply your wellness budget by 1.35 (ex: If your Wellness Budget is $1,000, your Fundraising Goal would be 1000 x 1.35 = $1,350.)

  • This helps us determine the general look and layout of your fundraiser page.
  • Identify your fundraising campaign Team Lead(s)
    The Team Lead(s) will have administrator access to the fundraising page, allowing them to edit campaign details and add/remove other team members. You must have at least one Team Lead and can choose to have up to three initially. More Team Leads can be added through the administrator panel on your Fundraiser page, once it is created.
  • Who will be responsible for your GO Dough spending card?
    Your GO Dough card will be embossed with your school name and shipped to your school address. If you have not yet identified who will be your GO Dough Cardholder, you may leave the field below blank and we will ship the card to your school (Attn: Your Name). Please note, you will need to identify a cardholder before you will be able to activate your card and access any funds.
